Consulting Agreement:
The Counseling Center at CELA agreement for consulting services form is to be completed and submitted before your initial scheduled consulting session. Do not submit an agreement for services form unless you have already scheduled your consultation. If you have any questions or need support completing the form, contact us for assistance.
